Here are some of my student evaluations.

They include all my ETH evaluations that were taken, as the format is more anonymous, and two special evaluations I asked at Stanford. One early on in my teaching carreer, to assess my potential problems, and the other one later when I was applying for job, so I had it in electronic form.
